Patient Presenting History
The patient presented reporting an 8 year history of headache and intermittent right (TMJ) jaw locking closed. She had seen a number of body workers from various backgrounds over the years reporting short term relief. The latest practitioner causing blistering inside her mouth by pushing too hard. The blistering had resolved by the time of her first appointment at Bangalow Headache Neck & Jaw Clinic.
Jaw pain was reportedly aggravated by yawning, biting mid to large sized food or chewing. Initial jaw opening was 20mm. Normal range jaw opening is approximately between 45mm to 55mm with individual variations.
Treatment commenced with some oral postural exercises and Roccabado jaw stretches with some gentle self massage of the masseter muscles.
After 4 treatments the patient was able to open her jaw 48mm and reported nil pain with any activity including biting, yawning or chewing. She was discharged with a set of home exercises.
When I saw the same patient for another reason 3 years later she reported having nil jaw issues since the 4th and last treatment for her jaw several years earlier.

Professor Mariano Rocabado’s 6 Exercises for TMJ Disorders
We often incorporate the 6 exercises prescribed by Professor Rocabado, emphasising caring for the head and neck to effectively treat TMJ disorders. Do these exercises six times daily, with six repetitions each time.
- Resting Position of the Joint: Place the tip of your tongue on the roof of your mouth and put gentle pressure on the palate.
- Controlled Opening: Open and close your jaw while gently pressing the tip of the tongue on the roof of the palate.
- Stabilisation of the TMJ: Place two fingers on your chin and open and close your jaw while keeping the tip of your tongue on the roof of your mouth.
- Stabilised Head Flexion: Lift and lower the chin as if nodding the head.
- Cervical Retraction: Bring your chin toward your neck (creating a double chin), then push it out again.
- Shoulder Retraction: Squeeze the shoulder blades together while lifting and lowering the chest.

Michael Hayward is a physiotherapist with 28 years of experience, including 24 years in private practice treating musculoskeletal injuries across the whole body. With a special interest in sports injury and headaches, Michael developed a keen interest in proven techniques for treating headache and migraine.
After successfully treating cervicogenic headache in his own practice for many years, Michael became interested in the Watson Headache® Approach and its success in treating migraine. He went on to complete all three Watson Headache® Approach training levels and is now a Watson Headache® Level 3 Certified Practitioner, qualified to assess the relevance of the upper neck in headache and migraine conditions.
